Privacy policy
How we handle your personal and health information, written so you can actually check whether we are doing what we said.
Top End Care is bound by the Australian Privacy Principles in the Privacy Act 1988 (Cth), by the Information Act 2002 (NT), and by the privacy obligations in the NDIS Code of Conduct.
What we collect
We collect only what we need to deliver and account for your supports:
- Your name, contact details, date of birth and NDIS number
- Your NDIS plan, goals and funded supports
- Health information relevant to your support, including medications, mobility needs, allergies and behaviour support plans
- Emergency contacts, and details of guardians, nominees or advocates
- Shift notes, incident records and progress records
- Information you send through forms on this website
Why we collect it
- To plan, deliver and roster your supports safely
- To claim payment through the NDIS, your plan manager or you directly
- To meet our obligations to the NDIS Quality and Safeguards Commission, including incident reporting
- To respond to feedback and complaints
- To recruit and screen workers
Who we share it with
We share your information only where you have consented, or where the law requires it. That can include your plan manager or the NDIA for payment, health practitioners involved in your care, the NDIS Commission for reportable incidents, and emergency services where there is a serious and imminent risk to life or health. We do not sell your information and we do not use it for marketing.
How we store it
Records are held in access controlled systems, with access limited to workers who need it for your supports. Paper records are held in locked storage. Participant records are kept for seven years after supports end, or until a child turns 25, whichever is longer. We destroy records securely after that.
Accessing and correcting your records
You can ask to see the information we hold about you at any time and we will provide it within 30 days at no cost. If something is wrong, tell us and we will correct it. If we disagree that it is wrong, we will attach your statement to the record.
Photographs
We will never photograph or film you without asking first. If you agree to a photograph being used, we will tell you where it will appear, we will put the agreement in writing, and you can withdraw it at any time without giving a reason and without it affecting your supports. We will then remove the image from anything we control.
Photographs of people on this website that are not of Top End Care participants or staff are licensed stock photography. They are illustrations of the kind of support we provide, not photographs of the people we support.
This website
This site is a static website. It sets no tracking cookies, runs no advertising or analytics scripts, and loads no third party fonts or resources. Information you submit through a form is sent to our form handler and then to our inbox, and it is not used for anything other than answering your enquiry.
Complaints about privacy
Contact us first if you are able to. If you are not satisfied, you can complain to the Office of the Australian Information Commissioner on 1300 363 992, or to the NT Information Commissioner on 1800 005 610.
